Wrong Cremation Blamed On Coroners Employee PDF Print E-mail

PITTSBURGH -- An employee in the Allegheny County Coroner's Office was to blame for a mix-up that resulted in the burning of a man's clothing instead of his body, an official said.

The coroner's office mistakenly sent Chad Mulvin's clothing to a funeral home for cremation and stored Mulvin's body in a morgue cooler after the 29-year-old was shot July 2, 2004.

The mistake was not discovered until April, when prosecutors asked for the clothes as they prepared for the trial of 32-year-old Joseph Larkin, the man accused of killing Mulvin.

The coroner's office determined that the employee did not follow policy, according to Chief Deputy Coroner Joseph Dominick.

"They did not look in the (body) bag," Dominick said.

Officials would not identify the employee, and Dominick would not say if the person was disciplined.

The office has a new policy to tag every body with two wrist bands, one of which will be removed and retained when a corpse is taken to a funeral home, Dominick said.

Mulvin's body was cremated in May, the coroner's office said.

Larkin will likely stand trial on a homicide charge in December.
